These Terms and Conditions (“Terms”) govern your access to and use of the VitalCare Corporation’s (VitalCare) website and related online services.
By accessing or using this website, you agree to these Terms. If you do not agree with these Terms, please discontinue use of the website.
Informational Purposes Only
Information provided on the VitalCare website is intended for general informational purposes and does not constitute medical, mental health, legal, or other professional advice.
Use of this website does not establish a clinician-patient relationship with VitalCare or any VitalCare provider.
A clinician-patient relationship is established only through VitalCare’s applicable intake, consent, and service processes.
Not for Emergencies
This website, general email addresses, online inquiry forms, and appointment-reminder systems are not intended for emergencies or urgent clinical needs.
If you are experiencing an emergency or believe that you or another person is in immediate danger, call 911 or go to the nearest emergency department.
Do not use website forms, email, or appointment-reminder messages to communicate urgent or emergency information.
Scheduling and Appointment Requests
Submitting an appointment request, inquiry, or other communication through this website does not guarantee an appointment or establish a clinician-patient relationship.
Appointments are confirmed only after confirmation from VitalCare or through an authorized scheduling system.
Clients remain responsible for complying with VitalCare’s applicable scheduling, cancellation, late-cancellation, and no-show policies.
Appointment Reminders
VitalCare may offer optional appointment-reminder and scheduling communications through telephone calls, voicemail, email, text message, or other electronic methods.
Appointment reminders are provided as a convenience. Failure to receive an appointment reminder does not cancel an appointment or relieve a client of responsibility for managing scheduled appointments or complying with applicable cancellation and attendance policies.
SMS/Text Messaging Terms
If you elect to receive text messages from VitalCare, you authorize VitalCare and service providers acting on VitalCare’s behalf to send communications related to your appointments and services to the mobile telephone number you provide.
Messages may include appointment confirmations, appointment reminders, scheduling information, cancellation or rescheduling information, and related administrative communications.
Message frequency varies.
Message and data rates may apply.
You are responsible for ensuring that the telephone number you provide is accurate and belongs to you or that you are otherwise authorized to receive communications at that number.
You may opt out of text-message reminders at any time by contacting VitalCare.
Your decision not to receive text-message appointment reminders will not affect your eligibility to receive health care services from VitalCare.
Wireless carriers are not responsible for delayed or undelivered messages.
Text messaging may not always be completely secure. VitalCare seeks to limit the amount of sensitive information contained in text-message communications.
Do not use appointment-reminder text messages for emergencies or urgent clinical concerns.
Email and Electronic Communications
If you communicate with VitalCare electronically, you acknowledge that ordinary email and certain other electronic communication methods may involve privacy and security risks.
VitalCare uses reasonable safeguards appropriate to the communication method and applicable law.
Please use VitalCare’s designated secure systems when instructed to do so for communications containing sensitive health or clinical information.
Privacy
Use of this website is also subject to VitalCare’s Privacy Policy.
Health information created or maintained in connection with health care services is governed by VitalCare’s Notice of Privacy Practices and applicable federal and state privacy and confidentiality laws.
